Services

Our service offers an environment where residents can be supported to reach their potential for independent living. The 24 hour staffed service provides support for social and personal development. Enabling the resident to address issues and copying mechanisms which have or are inhibiting their ability to react their potential for independent living.

We will work with those who have developed negative coping mechanisms, strategies and/or habits which have unfortunately led to poor life choices, situations and/or behaviours detrimental to self or others.

SpecialistCare

An all-inclusive group of male residents with a history of mental health issues, some of whom may present with behaviours that challenge or may have health issues which are independently managed. The building has wheelchair access. The men are no longer ill enough to be in hospital but not ready to live on their own. They require social support and structure to develop independent living skills and to live safely in the community. Packages of support are tailored to the needs of the residents.Specialist Care Ltd does not provide nursing care. Residents are supported and encouraged to access universal healthcare services.

Model Of Support

  • All residents will have individual support plans to support their engagement in meaningful daily living activities developed with the resident.  This may include both core (shared) support and additional 1:1 support
  • Residents are encouraged to participate in daily living activities such as cooking and cleaning with staff support
  • There will be 24 hour, 7-day onsite staff support
  • There will be a day and night shift including handover time.  We will have set minimum staffing operational numbers on each shift.  This will be adjusted if required dependent on resident’s assessed needs
  • The residential home manager is available Monday to Friday
  • Every resident will have an individualised activity planner reflecting support required.  This will include core support and additional individual support hours (1:1) if required based on individual assessed needs
  • Specialist Care Ltd may engage in pre-placement work with residents to ensure compatibility with our service and to formulate person-centred support plans
  • There are times where some residents may require more 1:1 support.  Required support will be based on individual assessed risks and needs
  • There is a 24/7 on-call system covered by the home manager and directors to support residents and staff

Resident Inclusion

  • Residents have access to adequate and right information provided in easily accessible formats to support informed choices and decision making
  • The lives of our residents are valued and continually enhanced through community integration
  • Residents are fully involved and participate in planning and decision making regarding their lives and support needs
  • Each resident will be supported to develop and implement their meaningful individual community-based support plan (depending on their wishes and needs) around activities including shopping, education, exercise, day centre attendance, health, finances, leisure and cooking
  • Residents have unrestricted access to clean and safe communal facilities
  • Residents are supported to eat well, participate in choosing and cooking meals and have access to information/advice on nutrition and healthy eating
  • Residents have a sense of community living and learning from people with similar challenges and are supported to access community opportunities
  • Residents are respected and supported to maintain privacy and dignity
  • Residents are able and supported to develop and maintain friendships and relationships and to connect with communities of their own choice.  This includes contact and visits by and to friends and families
  • Residents manage their own affairs including correspondence and finances.  Those requiring support with managing finances will be referred for appointeeship or deputyship
  • Residents understand their risks and participate in reducing and managing safeguarding risks
  • Residents will have up to date and regularly reviewed positive behaviour support (PBS) plans in place
  • Residents are involved in regular reviews of their support needs
  • Resident’s independent skills are monitored and regularly reviewed
  • Person centred planned activities (developed with residents) are in place for all residents
  • There are safeguarding arrangements in place to protect residents and staff from avoidable harm and all forms of abuse
  • Residents are supported to attend healthcare and any other relevant appointments
